These rankings are indexes of health outcomes and factors related to health outcomes. The rankings provide a picture of a population's health and help communities identify starting points for improving the overall health of their members. The counties are ranked best to worst, 1 to 159. In general, rural counties tend to have worse health outcomes than urban counties.
